Organization Overview
Heart Health Organization is a cardiovascular health care organization with the primary mission of offering high quality, acute health care, and preventive medical care to all people who live in, work in or visit the Phoenix East Valley and its surrounding areas.
This care and service will be delivered in a caring and compassionate manner, focusing on meeting the needs of the community. Heart Health Organization services extend beyond inpatient and outpatient care to include assessing and improving the health care status of our local community.
The Organization strives to enhance its capabilities and to deliver health care services, within the scope of its resources, in a cost-effective manner. All care and services will be provided without regard to race, color, creed, national origin, age, sexual orientation or ability to pay.
H20 was designed to provide first-class service for Hospital-based Cardiovascular Services. The purpose of H2O is to enhance quality, patient satisfaction, and improve patient outcomes in an efficient and cost-effective care delivery process. Our goal is to outperform National benchmarks in Cardiovascular care and create regional recognition by providers and payors as a preferred provider.

Oftentimes patients experiencing a cardiac episode will go to a hospital emergency room where an evaluation is performed and an emergency room doctor begins treatment. The patient waits for the on-call cardiologist. What sets us apart is our around the clock care. An in-house cardiologist is available from 7 am to 9 pm, and an emergency room on-call cardiologist from 9 pm to 7 am, seven days a week.
You will not receive unneeded imaging tests.
The H2O team believes in providing care for today and for the future. When you are in our care, we minimize the imaging tests you receive. This prevents radiation and kidney problems and decreases the chances you have to stay longer or return to the hospital.
History of Heart Health Organization
Heart Health Organization is the first cardiovascular service line co-management company formed in Arizona. The company was founded in 2012 after a year of intensive planning and collaboration among 14 hand-picked independent practices and 34 specialty physicians, representing the best in Cardiology, Cardiovascular surgery, and Electrophysiology specialists. The company was formed to provide contracted management services to Dignity Health hospitals in the east valley, to improve the quality, efficiency, and integration of cardiovascular services with hospital administrative services. This innovative platform provided a streamlined and cohesive mechanism for improving performance while reducing costs.
The early success stories are remarkable. One example is having an onsite cardiologist in the hospital available in the emergency room, and for hospitalized patients to provide immediate services to heart patients at Chandler Regional and Mercy Gilbert Medical Centers. The result has been top performance in nationally tracked benchmarks for cardiovascular services. 100% of heart attack victims will have their heart arteries opened within 90 minutes of arrival to the hospital, with an average time of 60 minutes. Heart surgery outcomes are now in the top 5% nationally. Hospital complications and readmissions have been reduced since inception. Most importantly the hospitals and the doctors are working together to do what’s best for the patients, and quality of care and availability of the best specialists are now available in a way never before seen.
Organization Structure
Heart Health Organization is working closely in collaboration with the Chandler Regional Medical Center as well as Mercy Gilbert Medical Center. Its organizational structure is made up of five main committees including Executive Leadership Team, Utilization & Protocol committee, Quality committee, Strategy & Finance committee and Management Services committee that meets on a biweekly basis and provides guidelines and create policies in various aspects of management.
